Hospital fitouts and day surgery

Where a fitout stops being an interiors project and becomes a construction and licensing project with an interior attached.

Every other page on this site describes a fitout. This one describes a building classification, a mechanical engineering brief and a licensing process, which happen to result in a fitout.

That reordering is the whole point. On an ambulatory project the layout leads and the services follow. Here the classification and the mechanical design lead, and the layout is what fits inside what they permit.

Class 9a comes first

Under the National Construction Code, a health-care building where occupants receive treatment and may be unable to evacuate themselves is Class 9a. It carries a heavier set of requirements than a consulting suite: fire separation and construction, egress provisions, alarm and detection, structural fire resistance.

What Class 9a actually is and what triggers it is worth reading in full. Whether your scope triggers it’s a determination for a certifier or building surveyor on your specific facility, not something a builder should decide for you. Get it in writing before the lease, because a tenancy that cannot achieve Class 9a construction, or cannot achieve it economically, is a tenancy you should not sign.

Then licensing, which can dictate rooms

Day hospitals and day procedure services are licensed and accredited under state health legislation, and the requirements differ between states. They can require spaces you had not planned, set minimum dimensions, and govern how recovery is staffed and observed.

Establishing the pathway before design is not bureaucratic caution. It is the difference between a layout that gets licensed and one that gets redrawn.

Then the mechanical design

Theatre and procedure room ventilation is engineered, not selected from a catalogue. Air change rates, filtration, and pressure relationships relative to the corridor and to adjacent spaces, designed by a mechanical engineer and commissioned with documented results at handover.

This drives plant size and location, which in turn drives whether a tenancy works at all. Plant has to go somewhere with structural capacity, acoustic separation and a maintenance route, and on a tenancy fitout that place is frequently not available.

Then reprocessing

At this scale AS/NZS 4187 generally applies rather than the office-based 4815, and it is a more demanding document. The sterilising department is laid out for one-way flow across receiving, cleaning, inspection, packaging, sterilising and sterile storage, with tracking and traceability designed in rather than added.

Sterile stock storage is separate, controlled and away from the dirty end entirely.

Recovery, and the discharge path

Recovery bays need line of sight from the nurse station to every bay, oxygen and suction to each, and enough separation that a curtain is not doing all the work.

Then the route out. A patient who has been sedated should not walk back through a full waiting room, and should be able to be collected close to the exit. On the projects that get this right the discharge path was drawn as deliberately as the entry.
